A national survey will help NIOSH improve how human factors are evaluated in firefighter line-of-duty death and serious injury investigations

WASHINGTON — The National Institute for Occupational Safety and Health is seeking fire service personnel to complete a survey on line-of-duty death and serious injury investigations.

Each year, 60 to 100 firefighters die in the line of duty. At a department’s request, the NIOSH Fire Fighter Fatality Investigation and Prevention Program conducts independent investigations and issues reports with recommendations aimed at preventing similar incidents.

NIOSH is seeking responses from chief officers, company officers and firefighters, particularly those with safety officer experience. The voluntary survey takes 15 to 20 minutes, and responses will remain confidential. Participants may skip questions or leave the survey at any time.

The survey is available at https://redcap.link/9zk8k63w and will remain open through January 2027. Participants can save their progress and return later. NIOSH recommends using a desktop or laptop for the best viewing experience.
